Comments:
A mandated Renewal inspection was conducted today. There were 10 children (29 points) with 4 caregivers. Children were observed playing with puzzles, napping, hand washing, having lunch, reading books in circle time, and free play. Reviewed 4 staff files, 3 children's files, emergency evacuation log and supplies, first aid supplies, menu, attendance, and physical plant. Violations of the Standards are recorded on the Violation Notice. Description: Based on record review, the employee did not have documentation of a central registry finding within 30 days of employment. Evidence: Staff#1's documentation for Child Abuse and neglect could not be found. Provider stated she recently purged files and may have mistakenly thrown away results.

Plan of Correction: I will resubmit immediately
