- Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22 VAC 40-111-60 (children's records) and 22 VAC 40-111-120B (assistant/substitute provider records).
An unannounced monitoring inspection was completed on 2/14/19 from 10:05am until 11:30am. At the time of entrance, there were 7 daycare children, plus the provider's child, in care for a total of 19 points with the provider and one assistant. A sample of 5 children's records and 3 provider/assistant/household member records were reviewed. Children were observed participating in morning activities, to include music and movement, story time and arts and crafts. Restroom and hand washing procedures were also observed. First aid and emergency supplies, documentation of emergency practice drills and required family day home postings were reviewed. The provider does not currently administer medication but is MAT certified. Medication is not administered to children in care at this time and the provider stated there have been no accidents or injuries since the last inspection. One violation in the area of administration is identified on the violation report and was discussed with the provider during the exit interview.
Standard #: 22VAC40-111-100-A Description: Based on record review and interview, the provider did not ensure that documentation of a physical examination by or under the direction of a physician was obtained prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 9/18/17 and was in care on the day of the inspection. 2. The record for child 2 does not contain documentation of a physical examination. Child 2 has a date of enrollment of 9/18/17 and was in care on the day of the inspection. 3. The provider confirmed that the records for child 1 and child 2 were lacking documentation of a physical examination. Plan of Correction: The provider will ask the parents for copies of the most recent physical examination for child 1 and child 2, which will be placed in the children's records.
