Technical Assistance:
Due to the licensee's non-compliance with the Mandated Federal Crib Standards, the licensee must come into compliance and submit appropriate documentation to the licensing inspector within 30 days of receipt of the violation notice. The licensing inspector and the licensee discussed that non-complaint crib(s) in the home can only be used for the licensee's own children or children that reside in the licensed family day home. The licensee has identified to the cribs that will be used for children enrolled in the family day home.

Today an unannounced Monitoring inspection took place between 1:30pm and 4:15pm. There were 9 children present with the provider and 1 assistant for a total of 26 points within the supervision guidelines. The children were observed napping, playing freely and eating an afternoon snack (animal crackers and banana). A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not update the emergency plan at least annually. Evidence: The last documented review of the provider's emergency plan is dated 2/5/2018.

Plan of Correction: I'll update it.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain updated immunizations every six months for children under the age of two. Evidence: Child #2 is currently 17 months old and the most recent immunizations are from when the child was 9 months old. Child #4 is currently 12 months old and the most recent immunizations are from when the child was 2 months old.

Plan of Correction: I will check with Child #2's parents to get the most recent and Child #4 has an appointment tomorrow and I'll request an update from the parents.

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview, areas and furnishings of the family day home were not maintained in a safe and operable condition. Evidence: At the start of the inspection, Child #7 was observed sleeping in a pack-n-play. The provider stated that she knew the child should not be sleeping in the pack-n-play but the child wouldn't sleep on a cot.

Plan of Correction: I will put another child on the cot that might stay and I'll put this child in a crib.

Standard #: 22VAC40-111-570-A
Description: Based on observation and interview, the provider exceeded 16 points while working alone. Evidence: At the start of the inspection (1:30pm), the provider was alone with two 0-15 months olds (8 points), four 16-23 months olds (12 points), and three 2-4 year olds (6 points) for a total of 26 points. The provider stated that her assistant was on her break and would be back at 2:30pm. The provider thought that her husband working upstairs counted as being an assistant. Caregiver #2 arrived at 2:30pm.

Plan of Correction: I'll make sure that my other assistant stays until my second assistant arrives.
