Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 3 of 3 children's records reviewed were missing required information. 1) Children A and B were missing a provisions of emergency preparedness form. 2) Child C was missing the employer phone number of both parents. Plan of Correction: Forms will be placed into files today.
Standard #: 22VAC40-111-210-B Description: Based on record review, caregivers did not receive the required amount of annual training. Evidence: 1) Caregiver A had 15 of the required 16 annual training hours for 2018. 2) There were no training hours documented for the substitute provider in the year of 2018. Plan of Correction: I will update files once my husband arrives.
Standard #: 22VAC40-111-320-A Description: Based on observation, small electrical appliances were plugged in during the child care hours. Evidence: A coffee machine was not in use and was plugged in at the time of inspection. Plan of Correction: Corrected during inspection
