Technical Assistance:
Technical assistance was provided regarding the menu and updating children's records.

Comments:
An unannounced monitoring inspection was conducted from 10:15am to 11:25pm. There were 8 children (26 points) present with three caregivers. The children were observed playing with blocks, listening to music and listening to children's books on CD. Children were also observed being bottle fed and washing hands before lunch. Emergency drill records were current. 5 children's records, 2 household member records and 2 caregiver records were reviewed. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of immunizations for a child enrolled before the first day of enrollment. Evidence: Child # 1's first day of enrollment was 2/5/19. Immunization records were not enrolled for Child # 1 before the first day of enrollment.

Plan of Correction: The parent had given me the shot records but I cannot find them today. I will ask the parent to bring in another copy.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain subsequent tuberculosis (TB) screenings from each caregiver every two years. Evidence: Caregiver # 1's most recent TB screening expired on 1/18/19.

Plan of Correction: I will get the TB done today.
