Technical Assistance:
Discussed assistant/substitute provider records and VDSS Child Abuse and Neglect training.

Comments:
An unannounced monitoring inspection was conducted today from 1:00pm-3:15pm. There were 8 children (21 points) supervised by 2 direct care staff. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping and engaging freely. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 4 of 5 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 10/2/2017) record did not contain documentation of parental acknowledgement of the provider's emergency preparedness plan, liability insurance, and an annual review of emergency contact information. The most recent documented review of emergency contact information performed by the parent was dated 11/6/2017. 2. Child #2's (start date: 3/21/2016) record did not contain documentation of mom's work telephone number. 3. Child #3's (start date: 7/5/2017) record did not contain documentation of an annual review of emergency contact information performed by the parent. The most recent documented review performed by the parent was dated 7/7/2017. 4. Child #8's (start date: 2/19/2019) record did not contain documentation of either of the two required emergency contact persons, and parental acknowledgement of the provider's emergency preparedness plan.

Plan of Correction: I will get the missing information.

Description: Based on record review, 1 of 2 caregiver records did not contain documentation of 16 clock hours of training annually. Evidence: Caregiver B's record did not contain documentation of any training hours for 2018.
