Technical Assistance: resource for a 10 hour Pre Service training. also provides online training for child caregivers Please forward the one background search result within 10 days of this inspection. Recommendation for issuance will remain pending until receipt of the background search result.

An unannounced Renewal inspection was conducted this date. A total of 7 children were present (22pts) with Provider and 2 Assistants. Walk thru of the entire home was conducted. Observation of the all 3 areas where the children were napping was conducted. The home provides ample space and toys and equipment that were determined age appropriate for the children currently in care. Review of all topical ointments, diapering procedures and emergency procedures were completed. All 7 of the children were napping when the Inspector arrived and were transitioning to waking at the end of the inspection. A total of 6 children's records, all staff and all household records were reviewed. No medications on site today and no identified food allergies. The previous violations found at the licensing inspection last May were reviewed and corrective actions were verified. This inspection began at 11:50am and ended at 2:45pm. Standard #: 22VAC40-111-920
Description: Based on observation of the programming area, a menu for the current week was not posted. Evidence: A menu is not routinely posted in the provider's home or given to the parents of the children enrolled. A menu was not found on site during the inspection.

Plan of Correction: Provider was unable to print the menu. Provider will use a weekly menu posting with either a wipe off board or chalkboard.

Standard #: 22VAC40-111-960-E
Description: Based on observation of the provider's refrigerator, baby bottles were not labeled completely. Evidence: All baby bottles did not have the date labeled.

Plan of Correction: Provider unaware that bottles needed to be dated. Provider will require parents to date each bottle along with the child's name.

Standard #: 22VAC40-111-60-B
Description: Based on review of 6 children's records, 3 out of the 6 records reviewed were incomplete. Evidence: Child #1 and #2's records were missing the immunization records and health exams that were obtained at or prior to enrollment. Child #3's record did not contain an emergency authorization for emergency care.

Plan of Correction: Provider had all health documents as required but replaced the health documents with updated immunizations and health exams. Provider will maintain all documents for currently enrolled children.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, Provider did not obtain documentation that one child was adequately immunized prior to beginning are. Evidence: Child #3 began care on 12/3/18. The immunization record maintained is dated 12/26/2018.

Plan of Correction: Provider was unaware that immunizations are required prior to enrollment. Provider will require this document prior to beginning care.

Standard #: 22VAC40-111-170-A
Description: Based on staff record review, the provider hired one staff prior to obtaining a current TB screening. Evidence: Staff #3 was hired on 8/27/18. The documented TB screening was dated 2/18/19.

Plan of Correction: We will ensure that all TB screenings are obtained prior to hire.

Description: Based on review of all staff records, 2 out of 3 staff did not receive the required 16 hours of annual training. Evidence: Staff #1 received a total of 7 hours of annual training. Staff #2 received a total of 5 hours of annual training.

Plan of Correction: We will complete the 10 hour online pre service training.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the napping areas, two cribs were maintained in an unsafe area. Evidence: Two occupied cribs were located in a storage area that was cluttered and had objects within reach of the two infants (age 17 months and 22 months).

Plan of Correction: We will relocate both of these cribs immediately.

Standard #: 22VAC40-111-250-A
Description: Based on observation of the programming space, a hanging item was found within reach of a child under the age of five years. Evidence: A sound machine was placed within reach of Child # 6(age 2). This device has a long cord that could be accessed by Child #6.

Plan of Correction: We will relocate this cord out of reach of all children.

Standard #: 22VAC40-111-510-C
Description: Based on observation of nap time, two infants were occupying cribs that were not spaced at least 12 inches apart. Evidence: Licensing Inspector observed two infants occupying two cribs that were located in a storage area. The storage area did not allow at least 12 inches of space between the occupied cribs.

Plan of Correction: We will no longer use this area for napping.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on household record review, one household member did not obtain a central registry search. Evidence: HH #2 does not have a central registry search result on file.

Plan of Correction: We will request this search and forward the result to the licensing inspector within 10 days.

Standard #: 63.2-1720.1-B-2
Description: Based on review of all staff records, one background check was not obtained prior to hire. Evidence: Staff #3 began employment on 8/27/2018, their fingerprint background search results were not obtained until 9/13/2018.

Plan of Correction: Provider unaware that fingerprints were required prior to hire. All new staff hired will have fingerprint search completed before they start.
