Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection.

An unannounced monitoring inspection was conducted today, February 25, 2019 to determine the provider's compliance with licensing standards. The inspection was initiated at 2:00pm and concluded at approximately 3:30pm. In attendance were 6 children totaling 13 points being supervised by the provider. Children were observed eating lunch, using the restroom, watching television, and preparing for nap time. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children including restrooms were inspected with no hazards found. Today's lunch consisted of chicken & broccoli casserole, mixed vegetables, wheat bread, and milk.

The family day home had the required postings displayed on the wall upon entering the children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected. Five children's records and provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place log, parent agreements, emergency preparedness plan, daily attendance log, and other pertinent information.

Standard #: 22VAC40-111-80-B
Description: Based on the inspection of administrative records, the verification of a child's identity was not obtained. Evidence: During the inspection of five children's records, it was discovered child #4 did not have any verification of identity in the record. Licensing standards require proof of identity and age within seven business days of the child's first day of attendance.

Plan of Correction: Per provider: I will get the birth verification from the parent by the end of the week and email a copy to licensing.
