An unannounced monitoring inspection was conducted today from 10:15am to 11:30am. A total of 2 children [5 points] were present with the provider. A sample of 3 children's records, 1 caregiver record, emergency supplies, emergency drills, physical plant and injury records were reviewed. Children were observed actively engaged in imaginative play activities with the provider. Nurturing interactions were observed. Areas of non-compliance are documented on the violation notice. Standard #: 22VAC40-111-800-B
Description: Based on a review of records, an annual review of the emergency plan was not documented. Evidence: The most recently documented review of the emergency plan was completed on 12/2/2017.

Plan of Correction: Corrected during inspection; no updates to the emergency plan are needed at this time and a review was documented.

Standard #: 22VAC40-111-840-B
Description: Based on records reviewed, injury reports did not include the required information. Evidence: Injury reports for Child #1, #2 and #3 did not include actions to prevent reoccurrence.

Plan of Correction: I will use an updated template that includes this requirement.

Standard #: 22VAC40-111-320-G
Description: Based on a review of records, an annual inspection of the wood burning fireplace was not conducted. Evidence: The most recently documented inspection of the wood burning fireplace was conducted 1/19/2018.

Plan of Correction: A fireplace inspection is scheduled for next week.
