54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Discussed with provider the following: obtaining background checks for household member when member turns 18, annual trainings, medications and diapering.

An unannounced monitoring inspection was conducted from 10:20am to 11:50am. There were 11 children (29 points) in care of three caregivers. The children were observed playing with blocks, puzzles, balls and toy trucks. The children were also observed eating lunch that was brought from home. A walk through of the house was conducted. 5 children's and 3 caregiver records were reviewed. Violations were cited during this inspection and are listed in the Violation notice. Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with children's full names and dates. Evidence: 1. There were 2 infants present today. 2. There were 3 infant bottles that were not labeled with names and date for the day. 3. One bottle was labeled with the name but not the date.

Plan of Correction: We will make sure that bottles are labeled everyday.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical exam for a child enrolled within 30 days of attendance. Evidence: Child # 1's first day of enrollment was 1/8/19. Documentation of a physical exam was not obtained for Child # 1 within 30 days of enrollment.

Plan of Correction: I will ask the parents to bring the physical exam.

Standard #: 22VAC40-111-240-B
Description: Based on observation, recalled equipment was being used by children at the family day home. Evidence: A child in care was observed seated on a bumbo seat. The bumbo seat in use had been recalled as it did not have the safety restraints attached.

Plan of Correction: I will discard of the bumbo seat and it will not be used again.
