An unannounced monitoring inspection was conducted this date. A total of 8 children (26pts) with Provider and 1 assistant present. When the inspector arrived, infants who were awake were observed in cribs, two of the older children were looking at books at the table. Observation of a new crib was made and determined in compliance. Children were observed sitting on the floor and playing with books, equipment and toys. Some interaction with staff and children observed. A review of 6 children's records all staff was conducted. Previous violations from the last inspection in August were reviewed, and one repeated issue food regarding baby bottle labeling. This inspection began at 9:40am and ended at 11:45am. Standard #: 22VAC40-111-960-E Description: Based on observation of the Provider's refrigerator, baby bottles were not properly labeled. Evidence: Baby bottles for Child #5 and #6 were not labeled with today's date. Plan of Correction: I will remind parents that all bottles must be labeled with the date and along with the child's names.
Standard #: 22VAC40-111-50-A Description: Based on review of documentation and staff statement, a daily written record of attendance was not found. Evidence: Provider stated that the attendance sheet was not printed for this week. Plan of Correction: I will ensure that each week's attendance sheet is printed and completed.
Standard #: 22VAC40-111-80-A Description: Based on review of child records, 2 out of 6 records reviewed did not have a child's proof of identity. Evidence: Child #1 began care on 2/4/19 and Child #4 began care on 8/15/18. No documentation that the Provider obtained proof of identity within 7 business days. Plan of Correction: I will request this information from the parent today.
Standard #: 22VAC40-111-480-B Description: Based on observation, equipment was used by a child that was age and size appropriate. Evidence: Licensing Inspector observed Child #1 (18 mos) sitting in an infant bouncy seat. This piece of equipment is recommended for those who are unable to sit up independently and for those under 19 pounds. Plan of Correction: We will only use this equipment for infants and those under 19 pounds who can not sit up independently.
Standard #: 22VAC40-111-530-B Description: Based on observation of the cribs on site, tight fitting bottom covers were not found on two of the four cribs. Evidence: Two cribs with thin mattresses did not have tight fitting sheets. Plan of Correction: Will replace the mattresses with thicker mattresses so the sheets fit tight.
Standard #: 22VAC40-111-640-2 Description: Based on observation, cribs were used by children not for their intended purpose. Evidence: When the inspector arrived, 3 infants were awake and either standing or sitting in cribs for approximately 25 minutes. Children should only be placed in cribs for the intended use of sleeping. Plan of Correction: We will remove children from cribs when they are awake and relocate them in a safe area to play.
