Reviewed ideas for improved programming.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 9 children, totaling 25 points. Children's records, caregiver records, caregiver training, background checks, administrative records, emergency preparedness items, the physical plant, children's nutrition, and programming were inspected today. The children were observed playing with toys, engaging in creative play, singing songs, and reading stories. The children were to be served chicken, rice, mixed vegetables, various fresh fruit choices, and milk for lunch today. This inspection was completed between approximately 10:00am and 12:00pm. Standard #: 22VAC40-111-790 Description: Based on observations, it was determined that the provider has not posted all required emergency telephone numbers. Evidence: The provider recently began using a different level of the home for child care purposes. The provider has not posted 911 and the poison control phone number near the telephone on this level of the home. Plan of Correction: I will post it today.
Standard #: 22VAC40-111-960-E Description: Based on observations, it was determined that three infant bottles were not labeled as required. Evidence: Three infant bottles were not labeled with each child's name and date. Plan of Correction: The bottles will be labeled.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that the file for the assistant caregiver is not complete. Evidence: The file for the assistant caregiver does not include emergency contact information. Plan of Correction: I will ask her to fill out the form.
Standard #: 22VAC40-111-70-A Description: Based on a review of administrative documents, it was determined that one parent has not been provided all required written policy information. Evidence: The parent of child B has not been provided the written Medication Policy. Plan of Correction: I will ask the parent to sign the form.
Standard #: 22VAC40-111-90-A Description: Based on a review of children's health records, it was determined that the provider has not obtained an immunization record for each child who has entered care. Evidence: Child E entered care on 11/5/18. The provider has not obtained a shot record for this child. Plan of Correction: I will remind the parent to bring the shot record.
Standard #: 22VAC40-111-90-C Description: Based on a review of children's health records, it was determined that the provider has not obtained one updated shot record as required. Evidence: Child A is 3 years of age. The shot record in this child's file was completed at age 6 months. The provider did not obtain an updated shot record every 6 months until the age of 2. Plan of Correction: I will remind the parent to bring a new shot record.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver health records, it was determined that the provider has not obtained a repeat TB screening within 2 years since the most recent screening. Evidence: The provider's TB health screening expired on 1/31/19. Plan of Correction: I already have an appointment for a TB screening.
Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver training records, it was determined that the provider has not completed all training responsibilities as required. Evidence: The provider has not conducted Orientation Training for the current assistant caregiver who began employment ion 2/1/19. Plan of Correction: We will do the training this week during nap.
Standard #: 22VAC40-111-460-D Description: Based on a review of administrative documents, it was determined that the provider has not maintained documentation of each dog's rabies vaccinations. Evidence: Two dogs live in the home. There is no documentation showing that each pet's rabies certification is current. Plan of Correction: I lost the vaccination paperwork when I moved all of my child care files from the basement. I called the vet where I took them for their shots, but that vet has closed. I will take both of them to get new rabies shots this weekend.
