An unannounced monitoring inspection was conducted on March 5, 2019 between the hours of approximately 10:50 a.m. and 2:30 p.m. Eleven children were enrolled and eight children were in the care of two caregivers on this date; two residential children were also in care (25 points). Five children's records, three caregivers' records and one household member's record were reviewed and/or updated. No medications were on site/administered, as is the policy of this family day home. Facility documents, posted information and safety equipment (first aid kit, flashlight, radio and fire extinguisher) were observed/reviewed. Lunches were provided by the children's parents and snacks were provided by the program. Program observation was completed. Activities observed included: indoor play - toys; music; restroom, diapering and handwashing procedures; lunch; storytime; naptime and arrival of three children. Adult/child interactions, behavior guidance and daily routines/transitions were observed. The posted daily schedule and menu were consistent with the activities and snacks provided. Substantial compliance continues to be maintained; one exception to compliance was noted on the accompanying violation notice. Thank you for your time and assistance during today's inspection. Standard #: 22VAC40-111-180-A Description: Based on review/update of three caregiver records and discussion with caregiver #1 on March 5, 2019, the program failed to obtain a current tuberculosis (TB) screening for one caregiver. Evidence: 1. Update of caregiver #3's record, on this date, determined that an updated TB exam/screening had not been obtained (date of original screening - 2/08/2017). 2. Caregiver #1 verified the date of the original TB screening. Plan of Correction: An updated TB screening will be obtained.
