An unannounced renewal inspection was conducted today from 10:00am-12:30pm. There were 4 children (14 points) directly supervised by the provider. The home, 1 staff record, 4 children?s records, 6 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, singing songs, practicing letters, numbers, colors and eating snack. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-960-E Description: Based on observation, infant bottles were labeled with the child's name but without a date. Evidence: Five infant bottles were observed in the Family Day Home with each child's name but without a date. Plan of Correction: I will make sure all bottles are labeled with a date.
Standard #: 22VAC40-111-60-B Description: Based on record review, it was determined that children's records were incomplete. Evidence: Child#1's record is missing 1 of 2 required emergency contacts. Child#2's record is missing the mother's work telephone number. Plan of Correction: We will request the information from parents.
Standard #: 22VAC40-111-430-B Description: Based on observation, the family day home has pressure mounted gates installed at two stair openings in the programming area. Evidence: Pressure mounted gates were observed at the top and the bottom of the stairs, no hardware installed gate was in use as a protective barrier at stair openings. Plan of Correction: We will attach the gate to the wall with screws.
