Standard #: 22VAC40-111-40-A
Description: Based on observation, the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: 1. The Family Day Home has a licensed capacity of 5 children. 2. At the beginning of the inspection, at approximately 10:45am, the provider was caring for 7 children and 1 resident child, totaling 8 children. At approximately 10:55am, 2 children left with an adult. A total of 5 children and 1 resident child remained in care at that time.

Plan of Correction: She is my granddaughter, she's live with me.

Standard #: 22VAC40-111-60-A
Description: Based on observation and record review, the provider did not maintain records for all children receiving care. Evidence: 1. The provider stated she did not have a file for Child #7 and Child #8. 2. Child #7 and Child #8 were receiving care at the beginning of the inspection (10:45am) until leaving the Family Day Home at approximately 10:55am.

Plan of Correction: I don't have because I don't give care to them but I hope so this don't happens again.

Description: Based on record review, 4 of 6 children's records reviewed were not complete. Evidence: 1. Child #3's record does not contain documentation that proof of identification was verified. 2. Child #1's record did not contain an immunization record. 3. Child #1, Child #2, Child #3, Child #5, and Child #6's record did not contain documentation of a physical examination. 4. Child #1 and Child #3's record did not contain written authorization for emergency medical care. 5. Child #2's record did not contain documentation of the annual review of the child's emergency contact information. Child #2's last annual update was completed on 1/1/2018.

Plan of Correction: Already spoke with parents about this they will bring to me next week because they need make an appointment. The parent already fix it.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 4 of 6 children's records reviewed did not contain documentation that required information was provided to parents prior to the first day of child's attendance. Evidence: 1. Child #1, Child #2, and Child #3's records did not contain a signed acknowledgement that parents were provided with the parent agreement including the Family Day Home policies. 2. Child #1, Child #2, and Child #3's records did not contain a signed policy for administration of medication. 3. Child #1, Child #2, Child #3, and Child #4's record did not contain a signed liability insurance declaration.

Plan of Correction: The parents already fix it.

Standard #: 22VAC40-111-280
Description: Based on observation, bleach was not stored in an area inaccessible to children. Evidence: 1. A bottle of bleach was stored in an unlocked cabinet below the sink in the bathroom used by the children in care.

Plan of Correction: My husband already fix it.

Standard #: 22VAC40-111-570-A
Description: Based on observation of children in care, the provider exceeded the maximum 16 points per caregiver. Evidence: 1. At the beginning of the inspection, at approximately 10:45am, the provider was caring for 7 children and 1 resident child, totaling 8 children (20 points). At approximately 10:55am, 2 children left with an adult. A total of 5 children and 1 resident child remained in care at that time (16 points).

Plan of Correction: This will never happens the other children's come in when I'm working.
