Discussion was held on the topics of current enrollment, current staffing, and background checks for possible future assistant. Prior to their hire/start date, the fingerprint check must be completed and on file at the Family Day Home (FDH). Please use the same Virginia Facility ID number and Field Print code numbers originally sent to your established a point of contact last year prior to the Provider and household members obtaining their checks. If you no longer have this information, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884. A CPS check will also need to be sent out within 7 days of the hire/start date, a Sworn statement will need to be completed, and TB documentation obtained within 21 days of their hire/start date. Discussion that all background checks are now good for 5 years, this includes the Sworn Statement, fingerprint check, and CPS check.
The following inspection took place between the hours of 9 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Arlington, Virginia. At the start of this unannounced monitoring inspection there were a total of 2 children (6 points) with 1 caregiver. A total of 3 children?s files, 1 staff file, and 1 household member?s file was reviewed. The children were engaged in the following: toy play, diapering, hand washing, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 3 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (03/12/2019), documentation of a physical examination was not available for review for Child #3. 2. The start date for Child #3 was listed as )1/02/2019. Child #3 was in attendance on the date of inspection. Plan of Correction: Documentation of a physical examination shall be obtained and placed on file for Child #3.
Standard #: 22VAC40-111-70-A Description: Based on review, 1 of 3 children's files reviewed indicated that not all of the required documents were provided and completed by the parents prior to their child's first day of attendance. Evidence - 1. On the date of inspection (03/12/2019), the following completed documentation for Child #3 was not available for review: the Information to Parents form, the Policy for the Administration of Medications form, the Liability Insurance Declaration form, and the Provisions of the Emergency Preparedness and Response Plan form. 2. Blank copies for each of these forms were contained within the file of Child #3. 3. The start date for Child #3 was listed as 01/02/2019. Child #3 was in attendance on the date of inspection. Plan of Correction: All missing forms shall be completed and placed on file.
