Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and that fingerprint checks, sworn statements, and TB test will need to be obtained and be on file for Household Members #1 and #2 prior to their 18th birthdays.

The following inspection took place between the hours of 9:30 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Lovettsville, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (30 points) with 2 caregivers. A total of 11 children?s files, 2 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: toy play, movie time, music, diapering, hand washing, lunch time, nap, and other organized activities. Standard #: 22VAC40-111-780-B
Description: Based on review, 2 of 11 children's files reviewed indicated that the Provider did not annually review with the parent the emergency contact information (required in 22 VAC 40-111-60 B 2) to ensure the information was correct, and obtain the parent's signed acknowledgment of this review. Evidence - 1. On the date of inspection (03/15/2019), the following children had reviews old than one year: Child #6, and Child #11. 2. The current documented review on file for Child #6 was dated - 01/21/2018. The current documented review for Child #11 was dated - 01/19/2018.

Plan of Correction: These parents shall be contacted, information reviewed, and signed documentation of this review shall be maintained.

Standard #: 22VAC40-111-70-A
Description: Based on review, 2 of 11 children's files reviewed indicated that not all of the required documentation provided in writing was completed and/or filled out correctly. Evidence - 1. On the date of inspection, the Policy for the Administration of Medications form for Child #7, Child #10, and Child #11 was either not complete or completed incorrectly based on the Provider's current certifications. 2. The form for Child #7 was not available for review. 3. The form for Child #10, and Child #11, the Provider indicated that "I (or other caregivers) WILL administer ONLY non-prescription medication." Neither Staff #1 nor Staff #2 are certified in the Medication Administration Training (MAT). 4. The form for Child #11 was not signed by their parent.

Plan of Correction: These forms shall be issued and completed again.

Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 11 children's files reviewed indicated that the Family Day Home (FDH) did obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/15/2019), an updated immunization page dated within the last six months was not available for review for: Child #3 (immunization page on file was dated - 03/13/2018), and Child #10 (immunization page on file was dated - 09/11/2018. 2. On the date of inspection, based on information on file, both Child #3, and Child #10 were under two years of age.

Plan of Correction: Updated immunization pages shall be obtained for each child and placed on file.
