Comments:
An unannounced renewal inspection was conducted today from 10:50am-12:45pm. There were 3 children (11 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 8 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, napping and eating lunch. Lunches are brought from home. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Description: Based on observation, the provider did not ensure medication was locked. Evidence: In the bathroom utilized by children, household member's medication was stored in an unlocked medicine cabinet.

Description: Based on observation, the provider did not have a working portable battery-operated weather band radio. Evidence: The provider could not produce a working portable battery-operated weather band radio.

Description: Based on record review, the provider did not obtain documentation that children were adequately immunized prior to their first date of attendance. Evidence: Child 2 did not have immunizations in their record. (Child 2, Start date: 3/1/19)

Description: Based on observation, areas and furnishings inside and outside of the home were not maintained in a safe and operable condition. Evidence: 1) A toy riding horse on the playground was not placed on level ground was wobbly and did not have safety straps which poses a potential safety hazard to children. 2) While a child was sleeping in a bassinet swing, the swing was observed to be lopsided and leaning more to one side which poses a potential hazard to a child in the swing. 3) On the outdoor playground, one blue bucket swing was hanging lopsided which a child could potentially fall out of. This swing also does not have the safety straps attached. 4) One high chair did not have the safety straps attached as required by manufacturer guidelines.

Description: Based on observation, the provider did not ensure a child occupying a swing was safely and securely strapped. Evidence: Child 1 was observed in a bassinet swing without the safety straps being securely fastened. The swing bassinet was lopsided and leaning to one side potentially when the child moved the child could fall out posing a safety hazard.
