Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 6 children, totaling 13 points. Children's records, caregiver training, background checks, caregiver records, the physical plant, children's programming, emergency preparedness items, and children's nutrition were inspected today. The provider reported to the Inspector that no children's medications were on site today. The children were observed upon arrival, as well as actively engaged in a variety of activities, including: playing with toys, engaging in creative play, playing group games, and interacting with each caregiver. The children were to be served chicken and vegetable fried rice, bananas, and milk for lunch later today. This inspection was completed between approximately 9:45am and 11:00am. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for assistant caregiver A does not include 2 completed reference checks.

Plan of Correction: I will do the reference checks.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record D does not include the parent's medical insurance policy number.

Plan of Correction: I will ask the parent for the missing information.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training records, it was determined that the provider has not conducted Orientation Training as required. Evidence: The provider has not conducted Orientation Training for assistant caregiver A. This caregiver has been employed longer than one week.

Plan of Correction: I will do the orientation today while the children are napping.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver training, it was determined that the provider does not hold current First Aid/CPR certification. Evidence: The provider's First Aid/CPR certification expired on 1/3/19.

Plan of Correction: I will register for the next available class. I will email my Inspector to let her know the date of the class that I register for. In the meantime, my assistant does have current First Aid/CPR.
