63.2 Facilities and Programs
An announced 2 month follow up from the conditional license having been issued was completed today. The physical plant, 3 caregiver/household member files, 3 children's files, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:30-2:40pm There were 3 children, totaling 11 points, under the supervision of the Provider. Today, the children enjoyed free play, cleaned up for and ate lunch. The interactions between the children and caregiver was seen to be positive and constructive in nature. Today, the children were served chicken with spinach and rice, oranges and milk. The Provider is not currently certified in MAT (medication administration training). Standard #: 22VAC40-111-830-A Description: Based on file review, the Provider has not practiced evacuation drills each month children are in care. Evidence 1. The evacuation drill log has the most recent drill having been completed on 12/14/18. Plan of Correction: This will be done and recorded.
Standard #: 22VAC40-111-920 Description: Based on observations, the Provider did not have the menu posted in an area accessible to parents. Evidence 1. There was no posted menu at the FDH. The Provider showed me the menu, which had been filed away. Plan of Correction: The Provider will post this.
Standard #: 22VAC40-111-60-B Description: Based on file review, 1 out of 3 children's records does not have all required information. Evidence 1. Child #2 does not have 2 emergency contacts in case the parents cannot be reached and the policy number for the medical insurance information. Plan of Correction: The Provider will ask the parents to add this to the child's record.
Standard #: 22VAC40-111-250-E Description: Based on observations, there were balloons in an area where a child under five years of age was playing. Evidence 1. A 9 month old child was playing on the floor in the main playroom. There were 3 balloons on the floor near the child. The child was not observed playing with the balloons. Plan of Correction: The Provider immediately removed them from this area.
