63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 15 points. Children's records, adult/caregiver records, background checks, caregiver training records, administrative records, emergency preparedness items, children's nutrition, and the physical plant were inspected today. The provider reported that no children's medications were on site today. The children were observed playing with toys, engaging in creative play, and interacting with both caregivers. The children were to be served goat curry, rice, spinach, bananas, sliced grapes, and milk for lunch later today. This inspection was completed between approximately 9:30am and 11:20am. Standard #: 22VAC40-111-750-A
Description: Based on a review of topical ointment products, it was determined that one item was on site without the required information. Evidence: One diaper ointment product was not labeled with the child's name. In addition, the parent's authorization for the use of this product does not include the name of the product to be used.

Plan of Correction: I will make sure the forms are always filled out correctly. The ointment was labeled during the inspection.

Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that infant bottles were not labeled as required. Evidence: Three infant bottles were not labeled with the child's name.

Plan of Correction: I will remember to include the name when I label the bottles with the date.

Standard #: 22VAC40-111-980-A
Description: Based on a review of administrative documents, it was determined that the provider has not accurately completed all permission forms allowing her to take the children off the premises. Evidence: The provider routinely takes the children on a walking trip to the neighborhood park. The provider has not included the trip destination on the permission forms signed by the parents.

Plan of Correction: The forms will always be completed correctly.

Standard #: 63.2-1721.1-A
Description: Based on a review of background checks, it was determined that the provider did not complete one required background search prior to a new adult family member moving in to the home. Evidence: One adult family member moved in to the home in October, 2018. The provider did not have this person complete a fingerprint background search prior to moving in to the home. This person's fingerprint search was not completed until 1/22/19.

Plan of Correction: I did not realize the fingerprint needed to be done as soon as she moved here. I will remember that in the future.
