Discussion was held on the topics of current enrollment, current staffing, and training hours. The Provider has informed the Licensing Inspector that she is currently enrolled in courses to obtain her CDA. As a reminder, please continue to record all trainings and maintain a copy of each certificate for course work and/or classes taken. College transcripts are also an excellent method for tracking all credit hours obtained.
The following inspection took place between the hours of 1 pm through 3:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 6 children (13 points) with 2 caregivers. A total of 6 children?s files, and 2 staff files were reviewed. The children were engaged in the following: nap time, toy play, bathroom breaks, hand washing, drop in for after school, snack, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 3 of 6 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (03/26/2019), documentation of a physical examination was not available for review for the following children: Child #2 (start date - 05/01/2018), Child #3 (start date - 01/07/2019), and Child #6 (start date - 05/01/2018). 2. While Child #3 had no documentation, both Child #2 and Child #6 had growth charts. A growth chart alone does not encompass the totality of the documentation required for a complete physical examination. Plan of Correction: Documentation of a physical examination shall be obtained for all applicable children and placed on file.
