Comments:
An unannounced renewal study was conducted today from 10:40am-12:15pm. A total of 6 children [12 points] were present with 1 caregiver. Children were observed engaged in various indoor activities after returning from a walk within the neighborhood. Children played with homemade play-doh and legos. A sample of 5 children's records, 3 caregiver/household member records, emergency drills, emergency plan, emergency supplies, physical plant and outdoor play equipment were reviewed. Violations:
Standard #: 22VAC40-111-830-C
Description: Based on review of records and interview, documentation of shelter-in-place drills was not maintained. Evidence: There was no documentation of shelter-in-place drills during the 2018 calendar year. Provider stated that the drills were indeed practiced but were not documented.

Plan of Correction: I will document all shelter in place drills moving forward.

Standard #: 22VAC40-111-60-A
Description: Based on interview, a record was not maintained for each child in care. Evidence: There was no record available during the inspection for Child #1, who was observed in care.

Plan of Correction: I will have the mother complete documents needed for children who obtain drop in care.

Standard #: 22VAC40-111-210-A
Description: Based on a records reviewed, 1 caregiver records did not include documentation of the required 16 hours of annual training. Evidence: 1. There was no documentation of annual training available for Caregiver #1. 2. There was documentation of 12 out of the 16 hours of annual training required for Caregiver #2.

Plan of Correction: I will do a better job of documenting the annual training that I complete.

Standard #: 22VAC40-111-240-A
Description: Based on observations, areas outside the home were not maintained in safe and operable condition. Evidence: Areas of chipped paint and splintered wood, along with 1 warped wood plank were observed on the jungle gym outdoors.

Plan of Correction: I will have it fixed by the end of the month. In the meantime, I will keep the children off of it.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations, the most recent findings from the last inspection were not posted. Evidence: The most recent inspection was conducted on 12/11/2018. The findings were not posted during today's inspection.

Plan of Correction: I will post today's findings immediately.
