A complaint was received involving lack of supervision. Interviews were conducted on 3/4/19 and the complaint was determined to be valid. The investigation on 3/4/19 was from 9:30 a.m. to 10:50 a.m. The exit interview was conducted on 3/25/19 from

Description: Based on interviews with the provider , the assistant and information from law enforcement, it was determined that the provider failed to provide age appropriate supervision for a 3 year 11 month old child who was in her care. Evidence: On March 1, 2019, Child A was dropped off at the Family Day Home at approximately 6: 15 a.m. The child's parent put the child down to go back to sleep on his "bed". The parent spoke with the provider who was across the hall. There were two other children present and both were napping. The other children were three years and 13 months of age. At some point Child A got up from his "bed" and walked into the hallway and up the stairs. He went out the front door of the home. Child A was found by a school bus driver at the intersection of Foresail Cove and Aquia Drive at 6: 37 a.m. The child was barefoot. The bus driver notified Aquia Police and Stafford Sheriff. The child was taken to the Aquia Police station. They knew about the daycare in the area and took the child to the provider's home. At 7:00 a.m., they knocked on the door and notified the provider that they had Child A. The provider stated that she did not know that the child had left the home.

Plan of Correction: Since the incident, I have placed a taller gate and child proof gate in hallway. I also put a child proof knob on the handle of front door. I also have the parent put the child on his mat in the room I am in when he goes back to sleep. I informed the parent to let me know if an incident happens again at home and they didn't think to tell me until it happened here. All day care parents were notified.
