Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 21 points. Children's records, caregiver health records, caregiver training, the physical plant, children's medications, administrative documents, and emergency preparedness items were inspected today. The children were observed enjoying circle time discussions, taking a walk through the neighborhood, and eating lunch. The children were served turkey, mashed potatoes, corn, oranges, and milk for lunch today. This inspection was completed between approximately 10:40am and 12:15pm. Violations:
Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative records, it was determined that the provider has not obtained clear documentation of pick up/drop off procedures for enrolled school-age children. Evidence: Two school-age children arrive for care at the end of the school day. These children walk to the provider's home from the bus stop. Neither the provider nor her assistants walk with the children from the bus stop to the home. The provider has not obtained signed documentation from the parent indicating when the children officially enter care and are the responsibility of the provider.

Plan of Correction: I will create a clear statement today and have the parent sign it at pick up time.

Standard #: 22VAC40-111-90-C
Description: Based on a review of children's health records, it was determined that one record does not include an updated immunization record as required. Evidence: Child D is age 6 1/2. The most recent shot record in her record was completed at age 2. There is no documentation of an updated shot record obtained between the age of 4 and 6 years old.

Plan of Correction: I will ask the parent to bring the updated shot record today.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that one caregiver has not submitted a repeat TB health screening within 2 years of the most recently completed screening. Evidence: The TB screening for caregiver B expired on 3/13/19.

Plan of Correction: I will make sure she goes for a repeat screening today during her break.
