Comments:
An unannounced monitoring visit was conducted today between the hours of 9:30am through 11:00am with the provider on site. There were 2 children (4 points) with the provider. A sample of 2 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, tv time and outside time to play with chalk. Interactions between the children and provider were positive. The physical space, evacuation drills and attendance records were reviewed. Standard #: 22VAC40-111-50-A
Description: Based on record review, the family day home did not keep a written record of children in attendance each day. Evidence: 1. The provider had a written record of children in attendance, but did not record the attendance for yesterday or today when the inspector arrived and the children had been at the providers home for more than 30 minutes.

Plan of Correction: I will make sure I take attendance each day.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: 1. The provider had a report of tuberculosis screening form dated 10/17/2016 and had been more than 2 years.

Plan of Correction: I emailed the physicians office to request a copy of the results during the inspection.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the family day home, inside were not maintained in a safe and operable condition. Evidence: 1. There was an area about 1 1/2 inches long on the stair wall that had chipped paint. There was also an area in the kitchen with chipped paint on the edge of the wall.

Plan of Correction: I will call and have someone repair it and paint.
