Comments:
An unannounced monitoring inspection was conducted on March 26, 2019 from approximately 11:40 am to 2:10 pm. Provider and assistant were present with nine children for a total of twenty-three points. Children were observed watching television, eating lunch and napping. Five children's records were reviewed. Provider and household member records were also reviewed.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation and interview, provider did not ensure bottles are labeled with the child's full name and date, if there is more than one infant in care. Evidence: On March 26, 2019, child #1 was observed drinking a bottle. The assistant verified the bottle was not labeled with the child's name and date. There were two infants in care at this time.

Plan of Correction: In the future, we will label the bottles with the name and date.

Standard #: 22VAC40-111-100-A
Description: Based on review, provider did not ensure 2 of 5 children's records have a physical examination within 30 days after the first day of attendance. Evidence: On March 26, 2019, the records for child #1 (start date January 2, 2019) and child #2 (start date December 10, 2018) did not have a physical.

Plan of Correction: Provider will ask parent for missing information.

Standard #: 22VAC40-111-60-A
Description: Based on review, provider did not ensure 2 of 5 children's records have documentation of required information. Evidence: On March 26, 2019, the following was found during review of children's records: 1. The record for child #2 (start date December 10, 2018) did not have two emergency contact names, emergency contact addresses, emergency contact phone numbers and person(s) authorized to pick up child. 2. The record for child #3 (start date August 6, 2018) did not have physician's address.

Plan of Correction: Provider will ask parent for missing information.

Standard #: 22VAC40-111-90-A
Description: Based on review, provider did not ensure 1 of 5 children's records has an immunization record prior to child beginning attendance. Evidence: On March 26, 2019, the record for child #2 did not have an immunization record. Child was observed in care on this date.

Plan of Correction: Provider will ask parent for missing information.

Standard #: 22VAC40-111-180-A
Description: Based on review and interview, provider did not ensure each caregiver and adult household member have an updated TB test or screening every two years. Evidence: On March 26, 2019, provider and household member have a most recent TB test dated August 27, 2016 which is more than two years old.

Plan of Correction: The screenings will be done this weekend.

Standard #: 22VAC40-111-210-A
Description: Based on review and interview, provider did not ensure caregivers complete at least sixteen hours of training annually. Evidence: On March 26, 2019, provider had documentation of completing two hours of training in 2018. Provider verified information.

Plan of Correction: Provider will start doing the training online.
