An unannounced renewal inspection was conducted today from 9:50am-12:45pm. There were 10 children (24 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing outside, napping and eating lunch. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Standard #: 22VAC40-111-830-C Description: Based on record review and interview, it was determined that emergency drills were incomplete. Evidence: Evacuation drills were not documented for the months of August 2018, and January 2019. Plan of Correction: We will make sure we do two drills when we do a shelter-in-place drill.
Standard #: 22VAC40-111-120-B Description: Based on record review, the provider did not maintain complete staff records. Evidence: Staff#2's record is missing documentation that two or more references as to character and reputations as well as competency were checked before employment. Plan of Correction: We will obtain two references today.
Standard #: 22VAC40-111-540-A Description: Based on observation and interview, it was determined that not all children occupying the feeding table were securely fastened with the safety strap. Evidence: Two children sitting at the infant feeding table were not using the chair's safety straps. When the licensing inspector asked why the safety straps were not being used, staff#1 stated that they forgot to use the safety straps. Plan of Correction: I have already talked to them many times and I'm going to make sure that they follow the rules.
Standard #: 22VAC40-191-60-B Description: Based on record review and interview, it was determined that a staff member did not complete a sworn statement or affirmation form prior to their first day of employment. Evidence: Staff#2 did not complete the sworn statement or affirmation form prior to their first day of employment. The provider stated that today (2/27/2019) is the staff's first day of employment. Plan of Correction: We will complete one today.
Standard #: 63.2-1721.1-B-3 Description: Based on record review, it was determined that there is no documentation that Child Abuse and Neglect checks were initiated for household members that have resided outside the state of Virginia within the last 5 years. Evidence: Household#4 has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained. Based on interview, the household member has been residing in the home for approximately 8 months. Plan of Correction: I will request one.
