Comments:
A renewal inspection was conducted with the provider. Two day care children were in care, total points was four. Both children were four years old. Children were observed during free play and snack. No children are on medications. All required paper work was posted. If you have any question about this inspection please call 540-272-9214 or e-mail lori.hancock@dss virginia.gov. The inspection time 3:00pm-4:10pm

Description: Based on review of two children's records not all had written permission to transport children in the provider's van. Evidence: There was not written permission from parents for the provider to take child off premises in the providers van.

Plan of Correction: I well have parents sign a permission to transport children.
