Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and household files. As a reminder, Household Member #5 will need a CPS check completed prior to his 14th birthday and placed on file for future review. The following forms were given to the Provider on the date of inspection: AUTHORIZATION TO APPLY A NON-PRESCRIPTION TOPICAL SKIN PRODUCT form, Policy for the Administration of Medications form, GENERAL PERMISSION FOR REGULARLY SCHEDULED TRIPS form, the VDSS MODEL WEEKLY MENU form, and RECORD OF EMERGENCY RESPONSE DRILLS form.

Comments:
The following inspection took place between the hours of 10 am through 2 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Falls Church, Virginia. At the start of this announced initial inspection there were no children in care with the 1 caregiver. A total of 1 staff file, and 7 household member?s files were reviewed. Children's files shall be reviewed once enrollment has begun. During today's inspection a tour of the FDH was conducted. The child care area shall be located in the basement of the home. The FDH was plenty of age appropriate toys and equipment for the children to enjoy. One electric fireplace was observed. One crib will need to be replaced (see violation notice), and all standards that remain undetermined shall be reviewed at the next unannounced monitoring inspection. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kits on site did not contain all of the required items. Evidence - 1. On the date of inspection (03/29/2019), the first aid kit on site at the Family Day Home (FDH) was missing the following items: scissors, tweezers, adhesive tape, and two triangular bandages. 2. The two first aid kits reviewed were a small blue kit and a mid size red kit.

Plan of Correction: All missing items shall be obtained and stored with these two kits.

Standard #: 22VAC40-111-790
Description: Based on observation, the emergency telephone numbers were not posted. Evidence - 1. The following required numbers were not posted by the telephone in the downstairs child care area: 911, the number for poison control, and the number of the person (along with name and address) to be used as an emergency back up. 2. The Provider confirmed this observation on the date of inspection (03/29/2019).

Plan of Correction: All required emergency numbers shall be posted by the downstairs telephone.

Standard #: 22VAC40-111-800-A
Description: Based on review, the Family Day Home (FDH) does not have a written Emergency Preparedness and Response Plan (EP&RP). Evidence - 1. On the date of inspection (03/29/2019), the FDH has not completed a EP&RP for the on site child care program. 2. The Provider confirmed this review on the date of inspection.

Plan of Correction: An EP&RP shall be completed and posted downstairs on a bulletin board with all of the other required postings.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area of the Family Day Home (FDH), inside, contained an area that was maintained in an unsafe condition. Evidence - 1. On the date of inspection (03/29/2019), a closet door located in a room in the downstairs child care area was observed off its track. 2. Door could possibly fall off and on to floor.

Plan of Correction: This closet door shall be repaired in order to prevent any possible hazard.

Standard #: 22VAC40-111-520-C
Description: Based on observation and interview, a crib of the Family Day Home (FDH), does not have supporting documentation that states that it meets the new safety standards set by the Federal Government, Consumer Product Safety Commission (CPSC) that went into effect on December 28, 2012. Evidence - 1. The date of manufacture on the FDH crib was listed as 05/2006. 2. The Provider stated (during an interview conducted on the date of inspection - 03/29/2019) that this same crib was used years ago when her children were younger.

Plan of Correction: A new crib manufactured after June 2011 will be purchased for the FDH. The existing crib shall not be used.
