Technical Assistance:
**Discussed fall zone clearance for indoor climbing equipment. The possible trajectory of falling children should be a factor in placing climbing equipment. **Dates should be included on bottles for infants in care.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am-12:30pm. There were 7 children (23 points) directly supervised by 2 staff. The child care area, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. There was an abundant supply of books, toys, and materials for the children. Interactions between children and caregivers was observed to be positive and constructive in nature. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-A
Description: Based on observation, one topical skin product was not labeled. Evidence: A jar of diaper ointment was not labeled with the name of the child it belongs to.

Plan of Correction: I will label it.

Standard #: 22VAC40-111-830-A
Description: Based on record review and provider statement, an emergency evacuation drill was not conducted monthly. Evidence: An evacuation drill was not practiced in February 2019.

Plan of Correction: I will resume monthly evacuation drills.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 5 children's records were incomplete. Evidence: Child 4 and 5's records do not contain complete information for 1 of the 2 required emergency contacts.

Plan of Correction: I will ask the parents to provide the missing information.

Standard #: 22VAC40-111-520-C
Description: Based on observation and provider statement, one of four cribs used for sleeping has mesh sides. Evidence: One crib used for sleeping in the sleeping room is a portable crib with mesh sides and does not meet the requirements for a sleeping crib.

Plan of Correction: I showed the director un-assembled cribs I have on site and will assemble one for use as soon as possible.
