An unannounced monitoring inspection was conducted from 11:10am-12:45pm. There were 10 children (25 points) in the direct care of 3 caregivers, within the supervision guidelines. The home, a sample of 5 children's records, 4 caregiver/household member records, the first aid kit, emergency materials, and evacuation drills were reviewed. There were no medications on-site today. The children were engaged in the following: eating lunch, doing sensory activities, playing with toys, toileting/diapering, taking a nap, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the Provider did not obtain documentation of a physical examination for each child prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #4 started on 5/31/18 and did not have documentation of a physical examination on file. 2. Child #5 started on 9/11/18 and did not have documentation of a physical examination on file.

Plan of Correction: We will ask the parents to bring it in within one week.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the Provider did not obtain a tuberculosis screening from each caregiver/household member at the time of hire or prior to coming into contact with children. Evidence: 1. Caregiver #1 began 1/17/2019 and did not have documentation of a current TB screening on file. 2. Household Member #2 did not have documentation of a current TB screening on file.

Plan of Correction: They will make appointments to get it complete in two weeks.
