An unannounced mandated Monitoring inspection was conducted today from 10:00 am to 11:30 am. There were 6 children (17 points) with 2 caregivers present. Children were observed playing with cars, putting together a puzzle, crawling and playing with manipulatives, washing hands, having lunch (rice, beans, meat and milk), and playing outside. Reviewed 2 staff files, 1 household member file, attendance, menu, cribs, emergency evacuation log, and physical plant. Violations of the Standards are recorded on the Violation Notice. Standard #: 22VAC40-111-180-A Description: Based on record review, the provider has not obtained subsequent TB screening every two years from the date of screening for caregivers and adult household members. Evidence: Staff#1 and Household member # 1 had expired TB screenings. (3/24/17; 3/23/17) Plan of Correction: I will send them to get new ones
