Comments:
The inspector conducted an unannounced mandated renewal inspection from 12:15 until 2:30. There were 8 children in care with one provider for a total of 19 pts. Children were observed eating lunch, playing with a variety of toys, being read to, and watching a video preparing for nap. Five children's records were reviewed and provider and household member records were reviewed. Six violations were corrected and there were four repeat violations. The provider does not administer medications. Please call your inspector at 276-698-9881.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, there was no physical examination for all the children. Evidence: 1. The inspector reviewed five records. One of five records (# 3) did not have a physical and had been enrolled longer than 30 days.

Plan of Correction: The child has been in care for a long time and the physical must have been misplaced, I will look for it and or get another one.

Standard #: 22VAC40-111-230-B
Description: Based on record review and interview, there was not documentation of annual training for all the caregivers. Evidence: 1. The inspector reviewed three records, #2 and #2 did not have annual training documented.

Plan of Correction: Caregivers will get the required training.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview, the provider does not have a land line. 1. The provider stated she does not have a land line.

Plan of Correction: An A.V request has been sent into the licensing office.

Standard #: 22VAC40-111-350-E
Description: Based on measurement, the water temperature in the bathroom that the children use is higher than 120 degrees. Evidence: 1. The inspector measured the water temperature at 123 degrees.

Plan of Correction: Provider will adjust the water to under 120 degrees or under.

Standard #: 22VAC40-111-520-C
Description: Based on observation, the provider did not meet the U.S. Consumer Product Safety Commission standards. 1. The inspector observe a crib and it had the manufacture date of May 2011.

Plan of Correction: The provider will tell the parent the crib needs to be picked up or the provider will dispose of it.

Standard #: 22VAC40-111-570-A
Description: Based on observation and interview, the caregiver exceeded 16 points and there was only one caregiver present. Evidence: 1. The inspector counted 8 children and there were a total of 19 points, the provider acknowledged she had 19 points.

Plan of Correction: My daughter was present until about 9:00 a.m. and will return in the afternoon. Will work with family members to make sure there are not too many points.
