Comments:
A Subsidy Health and Safety inspection was conducted on March 26, 2019 to review supplemental health and safety regulations. There were 9 children and 2 caregivers present. The children were observed during snack time, free play and a music activity led by the caregivers. A sample of records was reviewed, 5 children's and 1 caregiver. The first aid kit was observed in the FDH. Please refer to memo dated October 16, 2018 which defines the new Child Care Program regulation (22VAC40-665) which became effective October 17, 2018. (22VAC40-665-400.) Procedures for emergencies - Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be include in the emergency response plan. This will be reviewed at the next inspection by the LI. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on a sample review of children's records, the Provider failed to obtain documentation of a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The reviewed physical exam documented food allergies to milk and eggs for child #4. No written care plan from a physician was available for review in the child's record.

Plan of Correction: The Provider will inform the child's parent to request a written care plan from the child's physician. Documentation will be reviewed with all caregivers and retained in the child's record.
