Comments:
An unannounced monitoring inspection was completed on 4/5/19. There were six children present from four months through four years with the provider supervising. The point total was 15. Additional children were expected to arrive for the afternoon and an assistant would also arrive to assist the provider with supervising. Three newly enrolled children's records were reviewed, the provider's record, and two household member's records were reviewed. The provider did not have any medications. The inspector arrived at the provider's home at 8:10 am and departed at 11:00 am. The children were observed during arrivals and during free play time. There was discussion with the provider about the following: advance signatures and dates for children's records (five children's records were dated a year in advance for annual updates), the process for cleaning and sanitizing the diaper changing pad, liability insurance form for child #1.

Standard #: 22VAC40-111-50-A
Description: Based on observation and interview, the provider failed to ensure that there was a written record of children in attendance each day. Evidence: 1. The provider was observed completing the attendance for the week after the inspector arrived. 2. The inspector asked the provider if she had just completed this for the week. She stated that she was not sure where the attendance for the week was so she had filled out another one. 3. The two children observed in care when the inspector arrived were not on the attendance for Monday through Thursday. The provider was asked if the two children who were present at the inspection were not present all week. The provider stated that they were present this week. 4. Two children were observed to arrive at approximately 8:45 am. The inspector looked at the attendance at approximately 9:40 am and the two children had not been added to the written attendance. 5. A child arrived at approximately 9:45 am. The inspector reviewed the attendance at 10:40 am and the child had not been added to the written attendance. 6. There were six children in care at 10:45 am and there were three children who had not been added to the written attendance record who arrived between 8:45 am and 9:45 am.

Plan of Correction: The provider will make sure the attendance is completed as the children arrive and accurate for each day.
