Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and upcoming hire of new assistant (Staff #3). As a reminder, a fingerprint check will need to be obtained prior to her hire date. In addition, a CPS check from Virginia, an out-of-state CPS check from Texas, and sworn statement or affirmation, two references, documentation of an orientation training, and TB documentation.

Comments:
The following inspection took place between the hours of 10 am through 12 pm noon. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (including 2 resident children) (an altogether total of 25 points) with 2 caregivers. A total of 7 children?s files, 4 staff files, and 7 household member?s files were reviewed. The children were engaged in the following: toy play, games, music, story time, diapering, bathroom breaks, hand washing, lunch, infant sleeping, center activities within the child care area, and other organized activities. Description: Based on review, 3 of 7 children's files reviewed indicated that annually, the provider did not review with the parent the emergency contact information (required in 22 VAC 40-111-60 B 2) to ensure the this information was correct, and obtain the parent's signed acknowledgment of this review. Evidence - 1. On the date of inspection (04/03/2019), the following children's records did not have an annual review recorded as conducted: Child #2, Child #4, and Child #5. 2. The most current documentation on file indicating the last review conducted was - 03/29/2018 (for both Child #2, and Child #4), and 03/28/2018 (for Child #5).

Plan of Correction: The parents of all applicable children shall review this information, document this review as required, and (if needed) update their information.
