Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 3, 2019. The on-site inspection was from 10:55 a.m. to 1:55 p.m. At the time of arrival, there were seven children in the care of the provider and one assistant (18 points). There were a total of 16 children enrolled in the daycare. The exception to compliance was noted on the violation notice. The risk rating for the violation was noted on the supplemental page. There were five children's records reviewed, and three provider's/assistant's/substitute provider's records were reviewed. Two providers were interviewed. As is the policy of the provider, there were no medications being administered. Some items observed and reviewed during the inspection included: required posted information, first aid kit, emergency supplies, warm water temperature, fridge and freezer temperatures, daily attendance chart, menu, documentation of evacuation, shelter-in-place and lockdown drills, updated emergency preparedness plan, free play, dancing and movement with music, story time, outdoor play, bathroom and hand washing procedures, diaper changing procedures, lunch, naptime, and dismissal procedures. Standard #: 22VAC40-111-830-B
Description: Based on review of records and verification from the provider, there had not been two shelter-in-place drills practiced within the last year. Evidence: 1. The documentation of the shelter-in-place practice drills showed there were not two drills completed since March of 2018. The dates of the last two drills were 08/13/2018 and 03/15/2018. 2. The provider confirmed there had not been two shelter-in-place drills completed within the last year.

Plan of Correction: The provider will conduct a shelter-in-place drill as soon as possible and then will ensure that two are completed every year from now on.
