An unannounced renewal inspection was completed today. The physical plant, 5 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 11:50am-1:10pm. There were 6 children, totaling 15 points, under the supervision of the 2 caregivers. Today, the children cleaned up from eating lunch, listened to a story, enjoyed free play and prepared for rest time. The interactions between the children and Provider were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, pasta, grapes for the older children and banana for the younger child, berries and milk. Description: Based on observations and interview, the posted menu did not reflect the changes that the Provider made to what the children were served for lunches. Evidence 1. The Provider stated that what was on the posted menu for yesterday's lunch was swapped with what was to be served today (and vice versa). These changes were not made on the menu.

Plan of Correction: The Provider will change this.

Standard #: 22VAC40-111-70-A
Description: Based on file review 5 out of 5 children's records does not have all required information to be provided to the parents before the child's first day of attendance. Evidence 1. All 5 children's records have the front page of the Medication Policy form. However, the second page, which includes the parent's signature, is not in each child's record.

Plan of Correction: The Provider will give this page to the parents to sign.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 5 children's records does not have additional immunizations once every six months up to 2 years. Evidence 1. Child #4 is 3 years old. The most recent immunization record on file was completed when the child was 18 months old.

Plan of Correction: The Provider will ask the parents for this again.

Standard #: 22VAC40-111-280
Description: Based on observations, there were hazardous substances in areas accessible to the children in care. Evidence 1. In the bathroom that the children use, there was a bottle of clorox spray on the top of the toilet and a container of clorox wipes on the sink.

Plan of Correction: These were moved during the inspection.

Standard #: 22VAC40-111-320-B
Description: Based on observations, there were electrical outlets in areas accessible the the children in care without protective covers on them. Evidence 1. In the downstairs playroom that the children use, there were 2 outlets without protective covers in them.

Plan of Correction: Corrected during the inspection.
