Licensing inspector discussed required posting of documents.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:57am-2:50pm. There were 12 children (37 points) in the direct care and supervision of 2 caregivers. A sample of 7 children?s records, 2 caregiver's records and 4 household member's records were reviewed. The children were observed dancing, singing, reading, eating lunch, and napping. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contacts of the children at least annually. Evidence: 1) Child D's file was last reviewed 03/23/2017. 2) Child E's file was last reviewed 08/01/2019. 3) Child F's file was last reviewed 03/22/2017.

Plan of Correction: I can do it today.

Standard #: 22VAC40-111-940-B
Description: Based on observation, an operable thermometer was not available to monitor the freezer compartment temperature. Evidence: 1) The provider tried to remove the thermometer from the ice compartment. 2) The thermometer was not visible.

Plan of Correction: I will get it out and put it on the door.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's records were incomplete. Evidence: 3 of 7 Children's records were missing the following required information. 1) Child A was missing an emergency preparedness form. 2) Child B was missing date of enrollment. 3) Child C was missing a phone number of 1 of 2 required emergency contacts.

Plan of Correction: I can do it today.

Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider did not obtain a current report of tuberculosis screening for each caregiver. Evidence: Caregiver A did not have a tuberculosis screening on file.

Plan of Correction: She had it during your last visit. I will look for it and if I can't find it she will go get another one from the clinic.

Standard #: 22VAC40-111-530-E
Description: Based on observation, clean linens were not used each time a child rests on the bed. Evidence: 1) The pink linens on one crib and the green linens on another crib were soiled with a brown and white substance. 2) During nap time Caregiver A lied a child on each of the soiled linens.

Plan of Correction: It must have been from earlier, I will change the sheets.

Standard #: 22VAC40-111-570-A
Description: Based on observation and record review, the provider did not ensure that a caregiver does not exceed 16 points. Evidence: The provider and 1 additional caregiver were caring for a total of 12 children (35 points). The providers child added 2 points to the total which equated to 37 points.

Plan of Correction: Usually my husband is here but he had to do something.
