Comments:
An unannounced renewal visit was conducted today between the hours of 9:30am through 11:20am with the provider on site. There were 7 children (15 points) with the provider and the assistant. A sample of 5 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play and coloring. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on record review, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct and obtain the parent's signed acknowledgment of the review. Evidence: 1. Child 1 (last review dated 9/26/2017) Child 2 (last review dated 3/13/2018) Child 3 (last review 3/13/2018) and Child 4 (last review 4/02/2018) all need the parents to review the emergency information, make any updates and sign.

Plan of Correction: I will ask the parents to review, update and sign today.
