As a reminder, there should be no soft filled bedding or pillows used by children under the age of two years.

Comments:
An unannounced renewal inspection was conducted beginning at 12:45 p.m. and ending at 2:15 p.m. with the provider. There were three children in care ranging in age from 21 months to 3 years of age for a total of 7 points. Children were observed having free play. A three year old was observed watching age appropriate videos on a tablet. Childcare areas and supplies were observed. Three children's records, one provider record and five household member records were reviewed. Emergency drills were reviewed. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Description: Based on review of documentation, the provider failed to obtain documentation of a physical exam within 30 days of attendance. Evidence: Child #1, enrolled 9/10/2018, did not have documentation of a physical in the record.

Plan of Correction: The parent's will be asked for this information again. This information has been requested several times.

Standard #: 22VAC40-111-180-A
Description: Based on review of documentation, the provider failed to obtain documentation of repeat tuberculosis (TB) screening or tests every two years as required. Evidence: Provider #1, TB was dated 3/13/2017, Household #1, TB dated 3/13/2017, Household member #2, TB dated 2/26/2017, Household member #3, TB dated 3/13/2017, Household member #4, TB dated 2/27/2017.

Plan of Correction: TB tests will be updated as soon possible.
