Technical Assistance:
Reminders: - Assistant's CPR and First Aid training certification expires on 4/15. - Make sure that all required paperwork is completed for the Provider's 2 sons who will turn 18 prior to the next inspection.

Comments:
An unannounced renewal inspection was conducted today. The physical plant, 3 caregiver/household member records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected between 1:40-2:50pm. There were 7 children, totaling 20 points, directly supervised by 2* staff. *See Violation Notice. The Provider is certified in MAT, but there are no children with any medications needs at this time. Today, the children colored pictures, played outside, worked with play-doh, ate lunch and rested during naptime. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. For lunch, the children were served baked chicken, brown rice, zucchini, salad, whole wheat bread and milk. Violations:
Standard #: 22VAC40-111-520-A
Description: Based on observations and file review, the Provider did not have enough cribs for each child 12 months or younger to have one assigned to them. Evidence 1. There were 3 children under the age of 12 months, but there were only two cribs available for use at the FDH.

Plan of Correction: The Provider has ordered a third crib, which should arrive to the FDH next Tuesday, April 16th.

Standard #: 22VAC40-111-540-B
Description: Based on observations, an infant walker was used in the FDH. Evidence 1. During the inspection, a child was observed playing in an infant walker.

Plan of Correction: Once informed of this standard, the Provider immediately removed the child from the walker. This will not be used at the FDH.

Standard #: 22VAC40-111-550
Description: Based on observations, there was a play pen at the FDH being used for the designated sleeping area of a child in care. Evidence 1. A child was observed sleeping in a play pen during the designated rest time.

Plan of Correction: The Provider will not use this for children to nap in.

Standard #: 22VAC40-111-570-A
Description: Based on observations and file review, the Provider did not ensure that a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed. Evidence 1. Upon arrival to the FDH at approximately1:40pm, there was one caregiver supervising 7 children, which totaled 20 points. A second caregiver arrived back to the FDH at 2pm.

Plan of Correction: The second caregiver had a sick child that she went to care for, but immediately returned to the FDH. This was an unusual circumstance and will not be a common practice.
