Discussion on location of cribs if more than one infant is occupying a crib.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 10 children (23pts) one of which is a household member and not counted in the licensed capacity with Provider and 1 assistant. When the inspector arrived, the children were playing with toys in the programming area. One child was waking from morning nap. Children were observed transitioning from free play to singing and dancing to a video with staff. A counting activity was completed with the children and clean up of toys was observed. Observation of the crib space was made, only one child currently in care using a crib. Morning snack was provided which consisted of gold fish. A total of 5 children's records and all staff and household records were reviewed. Previous violations from the November licensing inspection were reviewed and corrective actions were verified. Standard #: 22VAC40-111-830-A
Description: Based on review of the provider's emergency evacuation drills, monthly evacuation drills were not completed. Evidence: No documented evacuation drill for the months of February and March.

Plan of Correction: I will make sure fire drills are done every month.

Standard #: 22VAC40-111-60-B
Description: Based on a review of 5 children's records, two records were incomplete. Evidence: Child #2 and #4's record was missing the first day of attendance and the liability insurance disclosure. Child #2 and #4's records did not have emergency contact information for two contacts.

Plan of Correction: These children are siblings, I will have the parent sign this information today.

Standard #: 22VAC40-111-70-B
Description: Based on review of children's records, two records did not have written acknowledgement from the parent. Evidence: Child #2 and #4's record was missing the parent's written acknowledgement of the provider's policies.

Plan of Correction: The children are siblings. I provided the information to the parent but the parent did not sign. I will have the parent sign today.

Standard #: 22VAC40-111-90-A
Description: Based on review of children's records, provider did not obtain documentation of immunizations for one child prior to enrollment. Evidence: Child #5 began care on 1/7/2019, immunizations that were maintained were obtained on 1/10/19.

Plan of Correction: I will be sure that shot records are obtained before they begin care.
