When at least one child receives care for compensation, all children, exclusive of the provider's own children and children who reside in the home, who are in the care and supervision of a provider, count in the licensed capacity. Annual staff training will be reviewed at the next inspection.

An unannounced monitoring inspection was conducted this date. A total of 13 children (31pts) with 2 staff present, which exceeded the licensed capacity of children. When the inspector arrived, the children were transitioning outside for some fresh air. They returned inside and selected activities to do independently such as wooden puzzles, books to read and playing with the ample toys available and within reach for independent selection. Observation of the programming space, bathroom and crib room was made. All topical ointments labeled with the child's name and remain current. The children lined up to wash hands before lunch. Staff were assisting with feeding the children and encouraging those that can eat independently. Lunch today was fish sticks, mixed vegetables and rice with peaches for dessert. Plenty of food available for second helpings. Positive interaction was observed between staff and children. A total of 5 children's records and all staff records were reviewed today. Previous violations were reviewed, corrections verified with the exception of one repeated violation regarding a health exam for one child. This inspection began at 10:20am and ended at 12pm. Standard #: 22VAC40-111-730-D Description: Based on observation of the bathroom, medications were not stored in a locked place. Evidence: Two bottles of cough syrup was found in an unlocked mirror cabinet located in the lower level of the bathroom. Plan of Correction: Both these medications were removed immediately.
Standard #: 22VAC40-111-100-A Description: Based on review of children's records, a physical examination was not obtained within 30 days of enrollment. Evidence: Child #1 began care on 6/17/18, a physical examination was not found for this child. This is repeated and remains uncorrected from the November licensing inspection. Plan of Correction: I will require a health exam from this parent by the end of this week.
Standard #: 22VAC40-111-40-A Description: Based on observation and staff statements, the provider did not ensure that the total of children in care did not exceed the maximum licensed capacity. Evidence: The provider has a licensed capacity of 12 children. A total of 13 children were in care. Plan of Correction: Two of the children are grandchildren of the provider. Provider did not know that relatives who are not considered household members were counted in capacity. One child is leaving care in June and no more than 12 children in care will be maintained.
Standard #: 22VAC40-111-90-C Description: Based on review of children's records, additional immunizations were not obtained once every six months for two children under the age of 2 years. Evidence: Child #1 and #2 are both under the age of 2 years. The last immunizations for each child was dated August 2018. Plan of Correction: Updated immunization documentation will be obtained from the parent.
Standard #: 22VAC40-111-520-C Description: Based on observation of the crib area, one mattress did not fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib. Evidence: The crib located on the right side of the crib room had a mattress that had approximately 3 inches of space between the mattress and crib. Plan of Correction: We will not use this crib with this mattress. We have an extra crib and mattress that will be used immediately. We will replace this mattress.
