Relating to required training hours annually.

Comments:
An unannounced mandatory inspection was conducted today from 9:00am until 11:15am. There were 11 children (25 points) present with 2 caregivers. The children were observed eating breakfast, playing with bubbles, playing floor games and coloring. There were an abundance of books and games for the children to choose from. Children's/staff records, emergency supplies, menu's, emergency procedures, and cribs and cots were inspected and observed. Areas of non compliance were cited and presented on the Violation Notice. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on review of the first aid kit, the provider failed to ensure that the first aid kit was complete. Evidence: The first aid kit was missing the following required supplies, tweezers, an ice pack, 2 triangular bandages and a first aid instruction manual.

Plan of Correction: Will purchase items for first aid kit.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to maintain up to date records at the family day home for each child enrolled. Evidence: Child #1's file did not have documentation of proof of immunization on the first day of attendance or proof of a physical by the 30th day of attendance.

Plan of Correction: Will get from the parent and place in file

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances such as, but not limited to cleaning agents were stored away from areas accessible to children. Evidence: A bleach/water solution was stored on the toilet tank in the bathroom.

Plan of Correction: Moved during inspection.

Standard #: 22VAC40-665-410-A-1
Description: Based on evacuation drill logs, the provider failed to ensure that evacuation procedures were practiced at least monthly. Evidence: There was no documentation that evacuation procedures were practiced during January 2019.

Plan of Correction: I will make sure to do the drills each month and write them down.
