Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children (14 points) with 1 caregiver. A total of 5 children?s files, 1 staff file, and 4 household member?s files were reviewed. The children were engaged in the following: learning activities, music, movement, toy play, diapering, bathroom breaks, hand washing, getting ready for lunch, and other organized activities. If you should have any questions, please call or send an e-mail. Thank you. Charlie Perkins, Licensing Inspector (703) 309-3963

Violations:
Standard #: 22VAC40-111-320-B
Description: Based on observation, a child-resistant protective cover was not installed on all unused electrical outlets accessible to children under five years of age. Evidence - 1. On the date of inspection (04/10/2019), protective covers were observed missing from the outlet above the sink within the downstairs bathroom. 2. This bathroom is off of the main child care area and is used by those children who are potty-trained and who are also under five years of age.

Plan of Correction: Corrected on the date of inspection. The Provider place a protective cover in the bottom outlet, and a room deodorizer in the top outlet.
