Technical Assistance:
Relating to organization of crib information.

Comments:
An unannounced renewal inspection was conducted today between 9:00am and 12:00pm. There were 7 children (24 points) with 2 caregivers. A complete inspection of the home, children's/provider/assistants records, evacuation drills, emergency procedures, emergency supplies, menu's and attendance records were reviewed. The children were observed listening to music, working with colors, napping and preparing for lunch. Handwashing was also observed. Areas of non compliance were cited in the Violation Notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of children's files, the provider failed to maintain up-to-date records at the family day home for each enrolled child.

Evidence:
1) Child #1's file did not have documentation information for a the second emergency contact or authorization for topical diaper cream.
2) Child #2's file did not have documentation of authorization for topical diaper cream.
3) Child #3's filed did not have documentation of names of persons other than the custodial parents who are authorized to pick up child or authorization for topical diaper cream.

Plan of Correction: I will get the forms from the parents and put in children's file.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface was cleaned and sanitized after each diaper change. Evidence: After a diaper change, the diaper pad was only sanitized and not cleaned with soap and water first.

Plan of Correction: Have made up a soap and water solution and will now clean then sanitize the diaper pad after each use.
