Technical Assistance:
During the inspection, the licensing representative discussed with the provider: - water temperature - menu - household members - annual updates coming up for review of children's record

Comments:
The following inspection was conducted from 12:15pm-1:15pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 8 children (24 points) with 2 caregivers. A total of 5 children's files, emergency drills and other documents were reviewed. The children were observed during outdoor play, snack, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-90-C
Description: Based on a sample review of records, 1 out of 5 children [under the age of 2 years old] were missing documentation of additional immunizations every six months. Evidence: The most recent immunization record for Child #2 [13 months old] was obtained 4/11/2018.

Plan of Correction: I will obtain the records from the parent.
