An unannounced monitoring inspection was conducted today from 9:20am-10:45am. There were 7 children (21 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing games, climbing, playing ball, reviewing colors, talking about sports, and watching a few minutes of a children's movie. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance o within 30 days after the first day of attendance. Evidence: Child 1 does not have documentation of a physical examination within 30 days after the first day of attendance. (Child 1, Start date:12/17/18) Plan of Correction: I asked the parents for the physical and I will ask them again for the records.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation that a child has been adequately immunized prior to their first date of attendance. Evidence: Child 1 does not have documentation of being adequately immunized. (Child 1, Start Date: 12/17/2018) Plan of Correction: The parents know to bring me the immunization record. I will ask them again for the records.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, the provider did not ensure staff submitted to fingerprinting. Evidence: Staff 1 did not submit to fingerprinting. (Staff 1, DOH: 4/8/19) Plan of Correction: The staff is here because my other assistant had a family emergency. I will make the staff member does fingerprinting.
